1. You must complete the form DS 82 ONLINE from the Department of State’s website. Click here for the Online DS82 application . Make sure that you print the application form after completing the online application.
Effective 09-26-05 the Department of State has made it mandatory that all passport applications first be completed online using the 2D bar code application which is available from the Department of State’s website
2. As a renewal passport applicant... for persons over the age of 16 years old the passport is valid for 10 years If your passport has not been
expired for more than 60 months, then you may submit the information directly to us without having to appear at the post office or county clerk. If it has been more than 15 years then you will need to obtain a new passport by going through the new passport procedure.
3. The following is a list of the information that you will need to send to A Rush Passport & Visa Services
A. Your most recent passport.
B. Two passport photographs.
C. Photo copy of your picture drivers license.
D. Proof of Name Change (if any )
* Original /certified marriage certificate or divorce decree from the clerk of the court showing
place and date name was legally changed
E. Letter of Authorization to transport documents (A.K.A Courier Authorization Letter)
2 copies are needed - 1 inside of the package and 1 outside of the
package
F. Proof of departure.
• For tourism: photocopy of airlines ticket or paid Itinerary.
• For business class service, a letter from your employer or prospective employer on company
letterhead signed by someone of official stature within the company other than yourself.

6. Use the provided FedEx air bill example to send the materials to A RUSH Passport & Visa Services.
Place the above mentioned materials in an 8x11 envelope. Place that Envelope into a FedEx envelope. We will process the materials and send the completed passport and materials back to you using a return FedEx provided by you.
