Q: Do I need to have an itinerary?

 

            A: Yes,

                Business:  You will need a letter on company letterhead giving details of the trip.

                Tourist:    You must show copy of airline ticket, itinerary or E-ticket.

 

Q: What if I have not booked my trip?

 

            A: Contact our travel division A Rush Travel Services for a quote on Airfare, Cruise and Hotels.

 

Q: Is your service guaranteed?

 

            A: Our portion of the service is guaranteed. However, we cannot control what the Department of State will do or not do and can we control third party shipping such as FedEx or UPS etc.

 

s Q: Do I have to use FedEx?

 

            A: FedEx is our preferred vendor for overnight services; you may use any other overnight company you choose. However, for Same Day processing the package must be in our processing location by 8am. You must have a tracking number for the package so we can track its location. 

 

s Q: How do I determine what address to FedEx the package to?

 

       A: You must first contact a document specialist at A Rush Passport for instructions and the correct address where to send the package.

 

s Q: Why do I need 2 copies of the Letter of Authorization and 2 copies of the travel itinerary?

 

  A: One copy of the Authorization letter and One copy of the travel itinerary goes inside the sealed package. The other copies go on the outside of the package. Our courier must present proof of departure and courier authorization in order to process your passport application

 

s Q: Do I have to fill out the application Online?

 

            A: Yes, The Department of State will not accept hand written applications requesting rush service. The reason that we are required to complete the application form is that once the application is created a 2-D barcode is implanted in left corner of the application form. This is a Data input Barcode that the Department of State can scan and transfer the data information directly to their computer system.

 

s Q: How do I change my name on my passport?

 

            A: If your passport was issued to you less than a year ago you should click on the section for change of name.  If it is more than 1 year you will have to renew your passport and send the original court document stating your name change.

 

s Q: I have been married and divorced multiple times. What name should I use on my application?

 

            A: You will need to apply for the passport in your current name.  Your Name on the application must match the name listed on your driver’s license or ID.

 

w Q: Do I have to list all the names all my previous names and dates of Marriage and divorce?

 

     A: YES you must list all other names that you have been know as

     and the dates that you were married and divorced in chronological order.

 

s Q:  My Passport has been lost or stolen, how can I obtain a new one?

 

            A: You can re-apply for your passport if you have a certified copy of your birth certificate. If you are unable to locate your certified birth certificate the department of state may accept certain other items sas proof of your birth. See www.arushpassport.com for a list of supplemental information that can be submitted. You should also complete the supplemental information questionnaire listed in the menu choices.

 

            If you cannot complete the supplemental ID questionnaire you can pay $60.00 extra to request a file search of your last passport record.

           

s Q: My passport pages are full, how can I add pages to my passport?

 

            A: You must complete amendment form DS-4085. Include a check payable to the Department of State for $60.00, A Rush Passport & Visa letter of authorization, a copy of the travel itinerary or airline tickets and your current valid passport.


You can add pages in your passport 2 times for a total of 72 pages in your book. After that you will need to renew your passport for more pages.

 

s Q: What would stop me from getting a passport?

 

            A: The following is a list of some of the things that stop you from obtaining a passport.

 

            Child support payments in arrears

            Back taxes

            Default on student loans

            Criminal conviction history

 

s Q: What are my payment options?

 

            A: The Department of State fee ($135-Adult or $120-child) must be paid with check or money order. The postmaster fee ($25) can be paid with check, money order or debit card. Our fees must be paid with credit card or money order.

 

s Q: Who is responsible for the shipping fees?

 

            A:  You the customer are responsible for shipping fees. The shipping to A Rush and the return shipping back to you. The return FedEx fee can be paid with a credit card or Fedex account number. FedEx will bill your credit card for the FedEx charges.

 


 

 Frequently asked questions (New/1st & Childs Passport)

 

s Q: Does my Child require a passport?

 

            A: Yes. Effective 02-01-08 all U.S citizens REGARDLESS OF AGE must prove their citizenship when exiting and re-entering the United States. (This includes infant children)

 

s Q: My spouse/ Childs natural father or mother can not come to the passport acceptance facility to sign the application. What do I do?

 

            A: Both the natural father and natural mother are required to appear with the child at the closest passport acceptance facility. If only one parent and be present the absent parent may complete Affidavit DS 3053 giving the other parent permission to obtain a passport for the child or children.

 

            The consent form DS3055 must be notarized!!!

 

s Q:  How long is a child’s passport good for?

 

            A: Children are issued passports which are valid for 5 years.

            The child is issued a passport each 5 years until the age of 16.

            Persons 16 and over are considered an adult and are issued a passport valid for 10 years.

 

 

s Q: Can I go to a notary to have my materials verified?

 

     A: No, You must appear at the closest post office or county clerk’s office that accepts passports and visit with the passport agent to have your signature witnessed on the computer generated application.

 

s Q: Where Can I find an acceptance location?

 

            A: On the A Rush Passport & Visa website home page you will find a menu. Click on the menu choice “Where to apply”. Enter your zip code. It will list all post offices and county clerks offices closest to your location that accept passports.
