Requirements to Expedite Name Changes on Valid Passport
1) Effective 09-26-05 the Department of State has changed the way they process Change of Name and other amendments to passports. As an additional security measure Passport Services is eliminating the amendment of currently valid passports. The form DS19 has been replaced by form DS-5504.
This form should only be used for making a data change for a previously issued, currently valid U.S.passport WITHIN ONE YEAR from the date of issue:
When there is a change in your identifying information, such as a name-change due to marriage or by court order;
To extend the validity of certain previously limited passports; or
To correct a Passport Services’ printing error.
There is no charge for routine service, which is receipt of your replacement passport within 6 weeks from the date you submit it.
If Expedited Service is requested for any of the above, with the exception of an error by Passport Services, there is an additional charge of $60 per application, plus the cost of two-way overnight delivery for receipt within 2 weeks from the date you submit your application.
Please follow the instructions on the form carefully. Complete, sign, and date the application.
Note: If it is MORE than one year since the date of issue, you must submit Form DS-82 (or DS-11, if appropriate) and pay the applicable fees.
2 ) YOUR MOST RECENT VALID U.S. PASSPORT TO BE REPLACED.
3 ) You must include your Marriage, Divorce, or Adoption papers that clearly show the name of the county and the dates that your name was officially changed.
4 ) If your name has changed more than once, you must construct a letter showing the chronological order that your name changed including the dates. (See Instruction Page 2 of the form)
5 ) New Photos. Since the passport agency will be issuing a new passport two recent passport photos will be required to be submitted with the application
6 ) A CHECK OR MONEY ORDER, Payable to U.S. Department of State for $60.00.
*As of 09-26-05 The U.S. State Department is requesting a $60.00 Expedite fee for any person requesting expedited services. Any one requesting Rush services (services obtained within 7 to 10 business days) must show proof of departure.
7)PROOF OF DEPARTURE
This can be in the form of copy of the international leg of the airline ticket, paid/confirmed itinerary or company letter from the employer or prospective employer on company letterhead.
2 copies are needed - 1 inside of the package and 1 outside of the
package
Tourist : Photo copy of the international portion of the airline ticket or Itinerary, which is marked paid or confirmed, showing the departure from the country within 10 business days.
Business : A letter form the employer on company letter head (see example)
This letter should be signed by someone in a management position (other than yourself).
8) AUTHORIZATION LETTER
Letter giving A RUSH Passport& Visa Services permission to process your application and pickup the completed passport. This letter allows A Rush Passport & Visa to become your agent and discuss the status of your application.
